Diabetes Community Health Profiles
YHPHO has become part of Public Health England, joining its Knowledge and Intelligence Team (Northern and Yorkshire).
The Diabetes Community Health Profiles are an invaluable tool to highlight key diabetes issues for each PCT. They will assist the diabetes community to identify health needs and plan services.

About the profiles
The Diabetes Community Health Profiles bring together a wide range of
data on diabetes in adults into a single source for the purposes of
benchmarking. The tool allows you to download a diabetes profile for
each PCT in England. Profiles are available for 2008/09, 2009/10 and 2010/11. It is designed
to provide an overview of the key areas of diabetes care and highlight issues
for further investigation. It can be used to benchmark against other PCTs and
provides a starting point for needs assessment work.
A series of Children and Young People Diabetes Community Health Profiles have also been produced for each PCT and those provider trusts which participated in the 2009 Royal College of
Paediatrics and Child Health Survey National Diabetes Survey.
