CVD: Primary Care Intelligence Packs

Data on prevention, detection and management for cardiovascular conditions to improve quality in primary care

Documents

Detail

The packs are a powerful resource for local conversations about quality improvement in primary care. For every clinical commissioning group (CCG) the packs give practice level data on prevention, detection and management across a range of cardiovascular conditions - high cardiovascular (CVD) risk and hypertension, stroke and atrial fibrillation, diabetes, kidney disease, coronary heart disease and heart failure.

Each condition has two components - benchmarked data and narrative. For key indicators, the data charts show the degree of variation but also identify the numbers of individuals in each CCG who are undiagnosed or inadequately treated.

Dr Matt Kearney, GP and National Clinical Director for Cardiovascular Disease Prevention said:

“The narrative is crucial and is framed to engage clinicians and commissioners in understanding the magnitude and impact of variation in diagnosis and treatment, and identifies important questions that arise from the data”.

“The packs will help clinicians and commissioners to ask themselves challenging questions about the potential for local improvement and to build the primary care leadership that is essential for driving a local CVD action plan”.
